La Porte Hospital Offering Physicals to La Porte High School Student Athletes for 2017-18 Season
La Porte High School will not be able to provide physicals at the high school as has been done in previous years. Athletes and their parents are reminded that physicals will still be required for summer participation including weight training and conditioning. The 2017-18 physical form may be picked up in the LPHS athletic office or by going to the Slicer athletic website at goslicers.com. Athletes are strongly encouraged to get their physical done before the end of this school year so that they are allowed to participate in summer activities. Physicals may be obtained at the following locations:
- Dr. Sarah Bancroft – 600 Legacy Plaza East, La Porte IN 326.2663 (Office open T, W, F until 5/18/17)
- Urgent Care Clinic – 508 Legacy Plaza West, La Porte IN 575.6244 (Open M-F 8am to 8pm; Sat & Sun 8am to 4pm) Can also wait on home with holding your place utilizing inquicker @ www.laportehealth.com and choosing “Online check – in”
- Our Primary Care providers – 400 Legacy Plaza West, La Porte 326.1775 (Open M-F 7:30am to 5:00pm)
