Community Health Survey Open to Lake and Porter County Residents

A survey designed by Community Healthcare System and Healthy Community Institute will give Lake and Porter County residents insight into the social and economic drivers of health. First, input is needed from the community. 

Community Healthcare System – which  includes Community Hospital in Munster, St. Catherine Hospital in East Chicago, St. Mary Medical Center in Hobart and Community Stroke & Rehabilitation Center in Crown Point – encourages residents of Lake and Porter counties to click the following link and share what they believe are the top issues facing our community: https://bit.ly/LPCCHNA

The survey also can be completed by scanning the attached QR code. The deadline for participating in the survey is Oct. 27.

Results of this survey, combined with other community data, will help organizations across the two counties better understand community health concerns, and the feedback will guide efforts for improvement. 

At no time in recent history has attention to the root causes of health been so critical. The COVID-19 pandemic has put a spotlight on public health. This comprehensive assessment takes the conversation beyond disease outcomes and explores the relationship between environment, behavior, illness and length of life. 

Survey results will give the community information about what is happening in their neighborhood. Conduent Healthy Communities Institute may invite members of the public to participate in a focus group or listening session.
