Center for Hospice Care Names New Manager of Bereavement Services

Center for Hospice Care (CHC) is proud to announce Jamie Jacobs, MSW, LSW, as the organization’s new Manager of Bereavement Services. Ms. Jacobs is succeeding Holly Farmer, MA, LHMC, who served the agency for more than 21 years.

Jamie will be an integral part of supporting CHC’s bereavement team. In part, she will be responsible for managing bereavement staff in all 4 of the Center for Hospice Care locations.  She will also oversee all bereavement services provided to hospice and community bereaved in all 9 counties that Center for Hospice Care serves.  Ms. Jacobs will be ensuring that every facet of the Center for Hospice Care’s Bereavement program is operating at the highest of standards.

With over eight years of Hospice experience, Jamie has carried several meaningful roles within CHC. Previous roles at CHC include Resource Bereavement Counselor and Camp Evergreen Director for 2 years; Bereavement Counselor for 3 years and Hospice Social Worker for 3 years. 

Jamie received her bachelor’s in social work degree from Olivet Nazarene University in 2010 and then her master’s in social work degree from IUSB in 2014.  Jamie was drawn into hospice work when she interned with CHC Social Work department in 2013.  She was also able to experience Camp Evergreen when she was an intern which, at the same time, introduced her to the bereavement aspect of the hospice philosophy which she explains is her passion. 

In Jamie’s spare time, she loves being outdoors and enjoys nature.  Her favorite place to visit is the Upper Peninsula of Michigan.
