Center for Hospice Care recently launched a new community-based palliative care program, Kaleidoscope, which offers a holistic approach to caring for patients with serious advanced illnesses wherever they call home. A team consisting of a nurse practitioner, palliative care nurse, community health worker, spiritual care provider and volunteers will visit patients in their homes to provide care.
Palliative care is a medical specialty that focuses on relieving pain and improving comfort for the patient as well as supporting the patient’s loved ones. Kaleidoscope is a wrap-around program that enhances the care provided by the patient’s attending physician. It offers expert evaluation of the patient’s needs and uses in-home visits to monitor their condition. The team approach is also effective at helping coordinate communication between the patient’s other doctors and care providers.
If you've been diagnosed with a serious advanced illness, you can benefit from home-based palliative care, especially if:
- You've experienced repeated hospitalizations or trips to the ER
- You're still seeking curative care
- You've experienced a decline in function and/or have had frequent falls
